Documentary Requirements:

- Brokers
- Two (2) recent 2 x 2 color photo
- Photocopy of valid PRC-issued Certificate of Registration and PRC-issued Professional Identification Card
(Originals thereof to be presented upon filing for comparison as faithful reproduction) - Photocopy of the Cash Bond or Surety Bond
(Original copy to be presented upon filing for comparison as faithful reproduction); and - Photocopy of the Official Receipt (Filing Fee for Registration)
- Specimen Signature (for new applicants)
- Salespersons (in addition to the above requirements)
- Photocopy Letter of Engagement/ Employment originally signed by an HLURB- registered Dealer or Broker
- Photocopy of proof of Broker’s DHSUD updated registration
- Business Firms (In addition to Letter A requirements)
- Secretary’s Certificate or Sworn Authorization of the Representative
- Photocopy of Certificate of Registration with t Security and Exchange Commission (SEC) for
Corporation/Partnership, Department of Trade and Industry (DTI) for Single Proprietor, or Cooperative Development Authority (CDA) Card (Original thereof to be presented upon filing for comparison as faithful reproduction) - Photocopy of the Articles of Incorporation, Partnership, or Cooperation Card (Original thereof to be presented upon filing for comparison as faithful reproduction)
- Current list of engaged or employed DHSUD brokers and/or salesperson;
- Dealer: (In addition to Letter A requirements)
- Photocopy of valid NBI Clearance or Court Clearance Card (Original copy to be presented upon filing for comparison as faithful reproduction)
